Instantiate Enrollment Flow Plugins

    Description

      Enrollment Flow plugins should be instantiated by attaching to Enrollment Flows, similar to Enrollment Sources (which are instantiated OIS Plugins attached to Enrollment Flows). As part of this, Enrollment Sources in Authenticate mode (and maybe other modes?) could be merged into this, maybe even eliminate Enrollment Sources entirely.
