Attribute List in Enrollment Attributes Configuration is flat

The attribute list in Enrollment Attributes configuration view is a flat drop down list. This causes no functional problems, but it is very unfriendly for the user.

In this ticket, we propose a simple UI improvement based on html select optgroups. This approach will create an one level grouping. The groups will  be:

• Co Person
• Co Person Role
• Organizational Identity
• Petition

This is a step towards making the UI experience friendlier for the end user
