enrollment flow stalls in Approved state when identifier assignment fails

To reproduce configure an identifier assignment that is destined to fail. For example, configure one identifier assignment to depend on another one but have the order reversed so that the value for the dependent assignment is not available because it is being processed first.

Then proceed through a self-signup enrollment flow with approval.

After the petition is approved the CoPerson record will be set to the Approved state but not the Active state and there will be no error or warning recorded or displayed at any time.
