Improve errors for misconfigured enrollment flows, or don't allow them to happen

      Description

      If an enrollment flow does not have a proper set of attributes defined, trying to enroll someone generates the unhelpful message "Please recheck your submission". Instead, an error about the undefined attribute should be generated (eg: org identity affiliation not provided).

      Better yet (and in addition), Enrollment Flow creation should pre-define required attributes and not allow them to be deleted.
