Deleting Extended Attribute Does Not Delete From Enrollment Flow

    Description

      If an extended attribute is deleted, enrollment flows that referenced it continue to reference it. eg, it will be listed at

      /registry/co_enrollment_attributes/index/coef:5

      but render "Notice (8): Undefined index: x:renewal [APP/View/CoEnrollmentAttributes/index.ctp, line 66]".

      The attribute is no longer displayed in existing petition records (although it may still be in cm_co_petition_attributes), which is probably not right.
