When a provisioner failsafe is triggered and the email is configured to be sent it does not send the email. I am unable to find a log of where it is supposed to have sent the email. We have implemented a work around in the meantime but would love to have this feature working for future use. 

Of note: Loader failsafe triggered notifications are working it is only provisioner notifications that appear to not be working. Discussed this with Chris on 08/29/24 and he said he was able to identify the issue.
