UI Items missed for Common Attributes page in Edit Flow

Description
"Metadata Provider Name (Dashboard Display Only)" field is required, but does not have red asterisk, and Save button is not disabled when the field is empty.
 
Steps

1. Log into UI
2. Click 'Add New' button and select ‘Metadata Source'
3. Create a Metadata Source, save
4. Go to the dashboard and click the edit link for the source you just created
5. Click on the 'Common Attributes' link on the left navigation list
    
   What is occurring:

• There is no asterisk to indicate that "Metadata Provider Name (Dashboard Display Only)" is required
• Save button is not disabled if there is no text in this field.
   
  What is expected
• Red asterisk at all times to indicate required field
• Error message below this text field when there is no text
• Save button disabled until requirement met
• Red indicator on the 'Common Attributes' item in the navigation list
