UI: Enable Provider Checkbox Missing in Edit Flow

Description

Steps

1. Log into UI
2. Click 'Add New' button and select 'Metadata Provider’
3. Enter required fields and be sure to NOT enable the provider on the create flow
4. Go to Dashboard and select the Metadata Providers tab
5. Click the edit button for the provider you just created
6. Access the 'Common Attributes' tab

What is occurring:
There is no enable button on this page. This prevents the user from changing the status of the provider from enabled to disabled/disabled to enabled in the edit flow.

What is expected
The user should be able to change the enable/disable status of the provider using a checkbox on the Common Attributes page.

Note: Wireframes do not include this checkbox. I am adding a subtask to update the wireframes.
