Error Modal Missing

Description
"Save your information" modal no longer displays.
 
Steps

1. Log into UI
2. Click 'Add New' button and add a new source, filling out a few pages, not saving
3. Click 'Add New' and select 'Metadata Source' or 'Metadata Provider'

What is occurring

The "Save your information" modal no longer displays to let user know their changes will be lost

What is expected

User should be warned that their entries will not be saved.
