Place a label to the right of all standard checkboxes in the form-list

The current form output places checkbox labels in the fieldName div in the left column of the form-list structure. While this is correct behavior for most form elements, it is not correct behavior for stand-alone checkboxes where the label should always be to the right of the form element itself to provide a larger and more obvious click target for the user.
