More than one error in provisioning leads to a greyed-out continue button

If manual re-provisioning throws more than one error, it is impossible to click the "Continue" button the second time since the whole modal dialog box is greyed out. You can get out of this by hitting return, but that cancels the provisioning. This may be encountered in GitHub provisioning if there is more than one user or group who doesn't map to GitHub.
