Make entries in provisioning log more identifiable

The provisioning debug log produces entries that span multiple lines.   Only the first line of each section identifies that provisioner the log lines are associated with.  If debug logging is enabled for multiple active provisioners, the log entries can be intertwined.

Please considered either adding a short greppable identifier to every line, or maybe allow per provisioner debug log files.
