Bug: Custom metadata source form is not advancable beyond first step when using attached yml

When using the attached yml to mirror the reported bug's configuration, I was unable to create a custom metadata source. The form validation will not let me progress past the first step. It is worth noting that the form validation appears to work fine with the default yml.
