Update tooltips on dynamic registration form

Scenario:

1. Login as an admin.
2. Click on the 'Add New' dropdown menu and select 'Add a new dynamic registration' option.
3. Place the cursor over any of the tooltips.

Issue: The tooltips are repeating the titles of the field.
