Actions Required Number not updating

Scenario:

1. User C has ROLE_USER and belongs to Group ZZZ.
2. Groups XXX and YYY are approvers for Group ZZZ.
3. Login as User C and create a Metadata Source. NOTE: It needs to be approved and enabled.
4. Login as the admin and click on the Actions Required tab. NOTE: I have 4 Action Required tasks.
5. Click the Approve button.

Expected behavior: The Actions Required number changes from 4 to 3. See attached screenshot.
Actual behavior: The Actions Required number stayed at 4.

NOTE: I tried refreshing my page, going to a different screen and returning to the dashboard, and logging out and logging back in. The number never updated for me.
