Metadata Provider: Metadata Provider Type Dropdown Should Not Be Editable

Description
Metadata Provider Type Field is currently editable (in Edit Flow). This should never become editable.

Steps

1. Log into UI
2. Click edit button for an existing 'Metadata Provider’
3. Select the Common Attributes page using the navigation on the left
4. Click on Metadata Provider Type dropdown

What is occurring:
User has the ability to edit this field.

What is expected
This field should never become editable
