      Tested in 1.0.1-SNAPSHOT-00299cd 
       
      Description
      There are two missing info icons and three missing item labels on the Edit Filter page.
       
      Steps (Prerequisite: Existing Metadata Provider)

      1. Log into UI
      2. Select the Metadata Providers tab from the Dashboard
      3. Click on the 'Manage Filters' button
      4. Click on the Add Filter button
      5. Compare this page to the wireframe in the parent ticket
         
        What is occurring:

      • Search By
      • Search Criteria by Entity ID
         
        Missing field labels for:
      • Search By
      • Search Criteria by Entity ID
      • Entity IDs Added
