UI: Required Fields Indicators Missing

Description
For Metadata Source on the Organization Information page, the following fields are required to move to the next page, but the red asterisks that called attention to this are now gone.

• Organization Name
• Organization Display Name
• Organization URL
   
  Steps # Log into UI

1. Click 'Add New' button and select '‘Metadata Source'
2. Move through the application to the Organization Information page
3. Enter text in one of the required fields.

 
What is occurring
The Next button (for page 3) is dimmed
 
What is expected
The Next button (for page 3) is dimmed AND there are red asterisks to show the fields are required as well as a message to state this.
