Modal Blocks Creation of Metadata Source

"Save your information" modal displays when user clicks next button.
 
Steps

1. Log into UI
2. Click 'Add New' button and add a new source
3. Enter Source Provider Name and Entity ID (OR click URL/Upload, OR Copy)
4. Click Next button

What is occurring

The "Save your information" modal displays and prevents user from creating a source
