Refactor Page Title Generation In Standard Views

    Description

      View/Standard/*ctp have ugly logic to populate the view page title. Refactor this logic into StandardController, with an ability to override on a per-model or per-controller basis.

      (In particular, Plugins can't work with the current setup without ugly hacking.)
